Kootenai Heart Clinics Northwest consists of a network of locations in Washington, Oregon, Idaho, and Montana. About the Congestive Heart Failure Department

The heart failure clinic is a specialty sub-clinic within Kootenai Heart Clinics Northwest. The clinical navigators work with inpatients, educating and helping them navigate their transition from facility to home and outpatient care. Navigators work at the bedside as well as in the clinic with a small team of experts in advanced heart disease management. Our goal is to provide the best patient care so they are able to achieve self-management, while reducing hospital readmissions.

Schedule/Shift: Four 10hr shifts a week.

About the Clinical Navigator (RN) Job

Kootenai Heart Clinics Northwest is seeking an experienced critical care nurse with knowledge of heart failure and chronic disease management. The focus of this job is on educating patients and helping to guide them through the health care system. As a Clinical Navigator you assist patients in understanding their diagnosis, treatment options and the resources we provide. You serve as an essential link between patients and all other medical staff. In this job you facilitate appointments for consults and support services within established service standards, including social workers, dietitians and physicians. The Clinical Navigator coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines. We get the opportunity to develop patient education programs and tools. You will follow patients throughout the care continuum, including inpatient admissions and collaborates with inpatient care management resources. We adhere to established productivity benchmarks. The Clinical Navigator is responsible for outreach efforts to establish and maintain positive working relationships with medical staff.

Requirements & Minimum Qualifications
·         Graduate of a nationally accredited Nursing program. 
·         BSN required.
·         As of February 1, 2017, MSN required or must enroll in a graduate nursing program within one year of hire and complete program by 2022.

·         Minimum of 3 years of relevant experienced required.
·         Knowledge of Microsoft Office required. 

·         State of Idaho Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the "multi-state privilege to practice" compact with Idaho.
·         Certification within specialty area required within one year of meeting qualifications.
