Improve the overall health of the communities we serve by managing patient care at assigned medical clinic as follows:
Education/Experience: Bachelor's degree in Nursing from four-year college or university required. Valid RN
Performs new patient intakes, including introduction to clinic, thorough documentation of history, and Quality Metrics review (immunizations, health maintenance, and chronic disease management metrics).
Serves as liaison between outside providers/health care entities and the clinic.
Collaborates with appropriate teams to design and test work flows and RN integration into the care team.
Performs medication reconciliation and prescription drug monitoring as needed.
Performs patient, family, and caregiver education including motivational interviewing and patient-centered health action plans.
Coordinates follow-up care for CHAS patients discharged from hospital.
Oversees and makes recommendations for improvement of clinic work flows.
Performs other duties as assigned.
license in the state of Washington required upon hire. Two years' nursing experience, preferably within a
community health setting, required. Experience with case management preferred. Experience with electronic
health records preferred.
Skills: Excellent customer service, oral and written communications skills required. Compassion for
patients required. Must be able to work well in team environment. Ability to multi-task in fast-paced work
environment, with ability to anticipate future needs required. Computer skills required. CPR/AED certified for a
Health Care Provider required.
Nurses are required to stand, sit, and be mobile one-third to two-thirds of the time.  They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms.  Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one-third and two-thirds of the day.  Communicating by talking / hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds.  Rarely is there a need to lift more than 26 pounds
