Frontier Behavioral Health (FBH) is a large, well respected community mental health ccenter located in eastern Washington. We are seeking experienced professionals to work within a team to provide integrated mental health services.
Care Coordinator - Health Homes Required - Master's degree in social work or the behavioral sciences with 1 yr year exp working in a behavioral healthcare setting, or Bachelor's degree in the behavioral sciences with 5 yrs of related experience working in behavioral health care or a health care setting. Some experience working with children is helpful.
Provide Care Coordination to individuals who have multiple chronic conditions, have been identified as medically vulnerable and are at risk for ongoing high usage of emergency care. Responsible for engaging with assigned members, assessing patient needs, providing education, support, coordination of care and referrals to community resources, with the goal of helping members attain or improve self-management of their conditions. Responsible for creating and reinforcing Health Action Plans to address social and chronic medical and behavioral conditions. Also responsible for coordinating referrals to community services and clinically indicated services such as social services, mental health care, substance abuse treatment and long term supports and services. Most work is off-site, typically in members' homes.
Crisis Response Specialist I Required - Master's degree in the behavioral sciences or RN licensed in the state of Washington. Must be a Mental Health Professional (MHP) per WAC 388-865-0150
Summary - Provide mobile and walk-in crisis intervention and evaluation services for individuals of all ages and families in emotional crisis as well as psychiatric emergencies. Conduct face-to-face functional assessments utilizing a multi- disciplinary approach when possible. Utilize the assessment to make treatment/case management decisions. Design and implement "viable less restrictive alternatives to hospitalization". Perform Spokane County Designated MHP functions required by the Involuntary Treatment Act. Perform DMHP functions and emergency consultations on a 24-hour rotating schedule. Actively pursue DMHP designation.
