Provides social work services to patients, families and identified caregivers as part of an interdisciplinary care team. This includes conducting psychosocial assessment, intervention and counseling with a particular emphasis on transitions of care in a healthcare setting. Maintains current knowledge of resources available and shares this information with patients, families and identified caregivers. Collaborates across the organization and with community stakeholders. Familiar with evidence based practices related to psychosocial assessment and intervention. Works under broad supervision. Performs other related duties as assigned.
Minimum Qualifications: Master's degree in Social Work required. Licensure and/or certification as required by the State of Idaho. Minimum of one year of social service experience in a health care setting or social services agency. Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.). Compliance with accepted professional standards and practices.
Essential Job Functions (Physical Requirements): Regular and predictable attendance is an essential job function. On feet a minimum of 8-12 hours, depending on length of shift. Read charts, dials, equipment settings, literature and other printed and handwritten materials. Communicate with patients, physicians, families and co-workers in person and on telephone. Work with equipment and manipulate equipment settings, computer keyboard, etc. Work environment involves minimal exposure to physical risks, such as operating dangerous equipment or working with chemicals.
Work Environment: · Kootenai Health operates 24 hours per day each day of the year; operational needs of departments may require that you be available to work all shifts, weekends and holidays to meet the needs of the patients. · Work is normally performed in a typical interior work environment.
